The use of gadolinium-based contrast agents (GBCAs) during pregnancy is generally avoided unless absolutely necessary due to potential risks. Gadolinium can cross the placental barrier and enter the fetal circulation, where it may remain in the amniotic fluid. Although adverse effects are not conclusively proven, there is some evidence suggesting a potential risk of harm to the developing fetus, including an increased risk of stillbirth or neonatal death when administered during pregnancy.

For this reason, gadolinium is classified as a category C drug by the FDA for use in pregnancy, meaning that it should only be used if the potential benefits clearly outweigh the risks. In cases where imaging is essential and non-contrast MRI cannot provide the necessary information, careful consideration and thorough consultation between the radiologist and the referring physician are crucial.
