Hot take! Your charting is resting on a false dichotomy, and it is muddling how we navigate our therapeutic communication.
There is a huge and crucial frame of reference that we are not used to recording, and that is the 'Practitioner Subjective'. Instead, we've been taught to construe such information as 'Objective' when it really isn't. This habit of ours, encouraged by SOAP charts, degrades our therapeutic trust and confuses our clinical teamwork.
