1. Duly Accomplished Personal Data Sheet (revised 2017)
2. Photocopy of Birth Certificate issued by PSA
3. Two (2) Pieces latest passport size ID picture with name tag
4. Original/Authenticated copy of Transcript of Records
5. Original/Authenticated copy of College Diploma
6. Original Certificate of Eligibility or Board Rating w/ PRC ID (Civil Service Professional, RA 1080, Penology Officer & PD 907)
7. Valid Clearances (i.e. NBI, Police, Barangay, MTC, RTC & Prosecutor's Ofc)
8. Valid Community Tax Certificate
9. Certificate/s of Previous & Present Employment
10. Certificate/s of Seminars/Trainings Completed
11. Marriage Certificate issued by NSO (if married)
