Step by Step Tutorial:

1. Double click convert it to layer 0.
2. Image - Canvas Size - Anchor choose top middle, change unit to percent, and Height change to 200%.
3. Ctrl + J to duplicate layer.
4. Ctrl + T - right click - Flip Vertical, and move it to bottom empty space.
5. Ctrl + J to duplicate layer again.
6. Right click on new duplicated layer - Convert to smart Object.
7. Filter - Blur - Motion Blur - Angle 90°, Distance 35-40 Pixels.
8. Ctrl + N to create a new work space, size doesnt matter.
9. Image - image rotation - 90° CW / CCW if your layout is not in vertical.
10. Filter - Noise - Add Noise - Amount 400%, Gaussian and tick Monochromatic.
11. Go to Channels and select red channel.
12. Filter - Stylize - Emboss - Angel 180°, Height 3px, Amount 500%.
13. Go to Green Channel, Filter - Stylize - Emboss, Angel 90°, others remain.
14. Activate all channels, go to layer, Ctrl + A select all, Ctrl + T to transform.
15. Right click - Perspective, Drag the bottom like i did.
16. Ctrl + Shift + S save it as PSD file at any location. (Named it as you like.)
17. Ctrl + W to close this window & back to previous work file.
18. On the smart layer, Filter - Distort - Displace, Vertical Scale about 30px, Stretch to fit, Repeat Edge Pixels, Choose the PSD file you did just now.
19. Create a new layer on top.
20. Gradient Tool - choose Black to transparent, Drag from bottom to middle.
21. Blending mode - Overlay, Opacity around 30%.
