UTair ATR-72 plane crash near Tyumen. Aircraft icing. April 2, 2012. The accident was caused by deficiencies in the organization of ground handling and personnel training at UTair-Technic. This was made possible by the lack of effective oversight by the UTair Technical Directorate and Inspection Directorate over compliance with the requirements of the airline's own ground handling and aircraft protection documents. This was coupled with the lack of oversight by the shift supervisor. This led to the captain and aircraft mechanic incorrectly assessing the aircraft's surface condition after an extended stay on the ground in intense ground icing conditions, and the aircraft being released for flight without treatment.
