The examination of a patient in physiotherapy is a comprehensive process that involves several steps to assess the patient’s condition, identify impairments, and develop an effective treatment plan. The key components are:

1. Patient History

Collect detailed information about the patient’s medical history, lifestyle, pain characteristics, and any specific complaints or symptoms.

Ask questions about the onset, duration, and factors that aggravate or relieve symptoms.

Understand the patient’s functional limitations and goals.


2. Observation

Observe the patient’s posture, movement patterns, gait, and any visible deformities or asymmetries.

Check for muscle wasting, swelling, or color changes in the affected area.


3. Palpation

Use hands to feel for muscle tension, tenderness, swelling, and temperature changes in soft tissues and joints.

This helps in identifying areas of discomfort, inflammation, or restriction.


4. Range of Motion (ROM) Testing

Measure both active (patient moves) and passive (therapist moves) range of motion.

Assess the flexibility and limitations in specific joints and note any restrictions or pain.


5. Muscle Strength Testing (Manual Muscle Testing)

Assess muscle strength on a scale, often 0 to 5, where 5 indicates full strength.

Test major muscle groups and compare the strength bilaterally to identify imbalances.


6. Neurological Examination

Test reflexes, sensation, and motor function, especially if there are nerve-related symptoms like numbness, tingling, or weakness.

This may involve checking dermatomes (areas of skin supplied by specific nerve roots) and myotomes (muscle groups).


7. Functional and Special Tests

Perform specific tests for different joints, like the Lachman test for ACL integrity or the Phalen test for carpal tunnel syndrome.

Functional tests assess the patient’s ability to perform activities of daily living (e.g., walking, squatting, lifting).


8. Gait Analysis (if relevant)

Evaluate the patient’s walking pattern to identify any abnormalities or compensations.

Analyze factors such as step length, cadence, and weight distribution.


9. Postural Analysis

Check for postural deviations that could be contributing to pain or functional issues.

Identify any misalignments, such as forward head posture or pelvic tilt.


10. Pain Assessment

Use pain scales (like the Visual Analogue Scale) to quantify pain intensity.

Determine pain type (sharp, dull, radiating), triggers, and patterns to guide treatment.


11. Assessment of Functional Limitations

Based on the findings, assess the patient’s ability to perform specific tasks or activities, noting where limitations exist.


After this examination, the physiotherapist will use the findings to develop a personalized treatment plan, set goals, and discuss them with the patient. Regular reassessment is also essential to track progress and make any necessary adjustments to the treatment plan.
