I don't know what happened between Medal of Honor and this game, but It wasn't good. The original felt smooth, evenly paced with a logical upramp in difficulty. FPS rarely dipped and levels were evenly pace. While the game could be difficult in some areas it always felt fair. MOH: Underground is the opposite of that in many aspects. This was not a fun playthrough by any means. The AI in this game can be incredibly cheap. Prefiring, shooting through walls, glitching through grenade blasts, enemies do not stop firing when they start and rarely miss and never reload. Some enemies (not the SS armor dudes) are able to take 3 shotgun blasts by the end of the game. Level pacing is mismatched and some later levels are incredibly easy, while some earlier levels are incredibly hard. There is no slow build up of difficulty, its all over the place. You are constantly running out of ammo because some enemies do not drop any. On the short levels there are tons of health packs laying around and enemies drop them, but on the Monastery Mayhem level the enemies don't drop health at all and barely any ammo. Weapons choices for levels seems like a mistake too, on the long range outdoor Mittelwerk level they give you a shotgun and an MP40, but on the Paris street fighting level they only give you a sniper rifle? What the hell is that? The weapon aiming is constantly glitching out and guns do not shoot where you aim, the original MOH had this problem but it wasn't nearly as bad. FPS drops bring the game to a PowerPoint crawl in some areas making it almost impossible to aim, meanwhile the enemies are banzai charging you firing nonstop from their laser guns. Not to be whiny but I had an awful time beating this game. Unpopular opinion, but I do not understand how people think this is a better game than the original MOH.
