Why is it that the actions that lead others to failure look so obvious in hindsight?

More importantly, why do we see the findings or contributing factors that led to a particular significant event as unique or different to that which might be happening in our own organisation?

Our paradigms, mental models and mindsets have a lot of influence on the way we see things, and they can prevent us from learning from serious events, particularly where our peers are singled out or shamed in the media for poor leadership or poor decision-making.

In this video I explore the psychology behind why your board might be missing red flags and how your board can be open to learning from significant events that happen in other organisations.
