FINDING THE BAD APPLE WON'T HELP YOU AVOID INCIDENTS // A LOOK AT COSTA CONCORDIA

When we try to solve issues of human error in our workplace using the bad apple theory, we often think that another procedure or new rule or even demotion or removal of that person(s) will fix the ‘issue’. But this approach does little more than make us feel good that we’re doing what is morally right; that is, we’re correcting a wrong, so it won’t happen again. The reality is, it won’t help you avoid future incidents or keep your workplace safe and it closes our mind off to really understanding the system in which we work, or for that matter safety.

In this video I talk about a common bias that can prevent you from really understanding safety and THE ONE QUESTION you want to ask yourself when you’re reviewing an incident investigation.
