Leviathan was added to Metal: Hellsinger as a horde mode. You need to complete 30 floors, which includes a new final boss that is specific to the game mode. Here's a high level overview of how it works:
The usual mechanics are mixed up compared to the main campaign. Enemies drop shards that you need to pick up, so you can gain access to the altar in the center of the arena. Once you have access, you choose from a random set of memories (3 by default, but 4 with an upgrade that you can buy later). These memories include one of the main game / DLC weapons that last for a set number of waves, offensive buffs, and defensive buffs. In addition, you can pick up Void Echoes, which are used to purchase permanent buffs for your character in the opening screen. And unlike the main game, you can bring every single powerup that you purchase into the Leviathan; however, this is where my main problem with this mode begins.
Everything is locked behind this new in-game paywall. This includes dashing and slaughters. In fact, you cannot purchase dashing and slaughters until you unlock them in Leviathan. Not only do you need to unlock them first, but you also need to gain enough Void Echoes to purchase them, which is a grind on its own. Core movement abilities that you have within the first 5 minutes of the main campaign are initially locked out to the player, which adds a completely unnecessary (and unfun) grind for the first couple of hours. And as a more general note, my other complaint comes in the form of getting unlucky and having no actual weapon aside from Paz or the sword for tougher waves. You can counter this by purchasing upgrades to ensure that weapons last longer and show up more often, but once again, you need Void Echoes to unlock these abilities, so enjoy that grind.
At the end of the day, if you want to play through this, then I strongly recommend focusing on purchasing dashing and slaughters as soon as possible. In addition, I would also grab the dash ability that stuns enemies for 0.8 seconds. You can stunlock enemies (yes, even elite enemies) while easily building ultimate meter and fury. After that, focus on getting the charge ability for Paz, weapon ultimates, and abilities that give you as much ultimate meter as possible. Regarding memories, your biggest priority should be around two bars of ultimate meter at all times. It saved my life multiple times in this run since Paz's ultimate is amazing. Leviathan is beatable without being an unstoppable killing machine through combat unlocks. It just may take some luck as well.
And as one final note, you clearly do not have to have perfect rhythm and aim to win this. My gameplay is proof of that.
In any case, if you read up to this point, then thanks for watching and feel free to drop by the stream:
