PCMH Annual Reporting
This webcast will share best practice and learnings from Open Door Community Health Center’s recent experience completing Annual Reporting for PCMH as part of a multi-site organization. Participants will understand the time commitment to complete submission, the importance of creating customized reports and leveraging your EHR to generate required data. Participants will learn about the importance of consistent monitoring and communication with Care Teams to ensure activities such as Care Plans, team huddles and pre visit planning are maintained throughout the "sustaining" period following recognition.
