Slope-Intercept form is y=mx+b. We want to find the values for "m" and "b".

Graph the two points that are in (x,y) form. Then find the slope by counting squares starting with the point on the left to find the rise over run (or you can use the numbers from the coordinates). The value for the rise over run is the slope which will replace the "m". The part of the line that crosses the y-axis is the y-intercept, which is the value that will replace the "b".
