In California the state is now giving incentive payments to Managed Health Care plans to work and contract with eligible nonprofits to provide these services. Check out this video where I show you the website and talk a little bit about the
Health care plans such as Cigna, IEHP, L.A. Cares, and many more are now contracting with nonprofits under the services of "community supports". Statistics have shown that when patients are supported socially and have basic needs such as housing, food, and medical care, they do better and live longer. the problem was that we were always segmented and did not work together. That is changing drastically because hospitals can only provide a limited amount of social services.
That is where you are going to shine because you have been building your capacity to do bigger and better things in your community. Check out your state or local initiatives to see if your region is following the trend. If not get with your local providers to see if you can become a vendor or even start a new initiative in your area.
