What most groups fail to realize is that people make mistakes all the time, it only becomes a problem when the process in place didn't catch it. This means that you should never stop your incident investigations at "human error." Keep pushing to figure out why the error occurred to find solutions that benefit the reliability of the task as well as the reliability of the person.
