In this video, we explain that 5-Why (or 5Whys) is a great tool that people tend to use too literally.

We see groups that end up using multiple root cause analysis tools because they only use 5Whys (stopping after the fifth Why) for small incidents, while other groups don't think it is worthwhile to put 5Whys together at all. These two extremes, which we often see, can be more limiting than they are helpful.

Here at ThinkReliability, we view building a 5-Why as a great starting point for any investigation.
