21 minutes of cutting-edge exosome science from the frontlines of research. Dr. Ed Park returns from the 5-day ISEV 2024 conference in Melbourne with insights from 1,000 PhDs across 52 countries. Leading researchers confess: "I was wrong about this." The field is only 13 years old, and the message is clear: we need liquid biopsies, we don't fully understand exosome synthesis, and the science is extraordinary.
The science of what we don't know. Exosomes are 100 nanometers—invisible to optical microscopy. They're confused with chylomicrons, VLDL, LDL, and apoptotic bodies. Particle counts aren't exosome counts. Ultra-centrifugation damages exosomes. Better methods: tangential flow filtration (TFF), density gradients, size exclusion chromatography (SEC/HPLC), polyethylene glycol (PEG) precipitation, and asymmetric flow field fractionation (AF4). Even Triton X100 can denature contents. Takeaway: avoid ultra-centrifugation.
Labs on chips and single-exosome detection. Researchers bypass the Abbe limit using electrical detection, optical detection, surface plasmon resonance, and Raman spectroscopy. Microfluidics, impedance interferometers, and solid-gold wells enable single-molecule detection. The technology is extraordinary.
Clinical breakthroughs. ExoDx Prostate IntelliScore: borderline PSA + exosome testing = no biopsy needed. Semen/saliva block dengue and Zika transmission. Retinal exosomes maintain eye homeostasis; absence implicates macular degeneration. MSC exosomes suppress lung fibrosis. In-ambulance stroke triage (hemorrhagic vs. ischemic) in development.
Liquid biopsies and cancer. Machine learning analyzes 4,000+ proteins and 270 small RNAs to predict brain cancer. Suppressing exosomal PD-L1 improved cancer survival ~20%. Checkpoint inhibitors (like Keytruda) work by blocking PD-1/PD-L1, unleashing T-cells to kill cancer.
Bottom line. Commercial exosome products often don't disclose true exosome counts—companies count particles, which are orders of magnitude more. Buyer beware. But the science? Extraordinary. The future? Liquid biopsies for cancer, stroke, and neurological disease.
