Webinar #21: Autoimmunity—When the Body Attacks Itself
Autoimmunity affects over 80 different diseases—from rheumatoid arthritis to lupus, multiple sclerosis to Hashimoto's thyroiditis. Yet most remain untreatable. Dr. Ed Park explores the immune system's staggering complexity and why exosomes might help where conventional medicine fails.

The Immune System 101:
The immune system has three jobs: fight critters (viruses, bacteria, parasites), fight cancer, and manage chaos (aging, entropy). It has two branches: innate (immediate, non-specific) and adaptive (learned, specific). The innate system uses natural killer cells, macrophages, and complement. The adaptive system trains T cells in the thymus and B cells in bone marrow to recognize specific threats.

T Cells vs B Cells:
B cells produce antibodies. T cells come in two types: CD4 (helper cells) and CD8 (killer cells). Both learn to recognize threats via MHC molecules—MHC-1 on all cells (says "kill me if I'm infected"), MHC-2 on antigen-presenting cells (teaches other cells what to kill). T cells are trained in the thymus to recognize "self" and not attack it. If they're too aggressive, they're killed. If they're not aggressive enough, they're killed. It's Goldilocks training.

Why Autoimmunity Happens:
The immune system sometimes forgets what "self" is, or a viral infection triggers cross-reactivity. Chronic inflammation, stress, and stem cell depletion all contribute. Over 80 diseases result—affecting every organ system.

Conventional Treatments:
Anti-inflammatory drugs (NSAIDs, corticosteroids)
Disease-modifying antirheumatic drugs (DMARDs): methotrexate, hydroxychloroquine, sulfasalazine
Targeted antibodies (difficult to fine-tune)
Immune suppression (risky, like organ transplant rejection)

Clinical Case: Dr. Park's Mom (83):
Young Park has rheumatoid arthritis. After 13 exosome injections (wrist blocks, individual knuckle injections, knees), she reports dramatic improvement: reduced pain, swelling, inflammation, and even some deformation reversal. Did exosomes change her rheumatoid factor or immune physiology? Unknown. But the phenotypic improvement is real.

Why Exosomes Might Work:
Shift macrophages from M1 (pro-inflammatory) to M2 (regenerative)
Shift T cells from Th1 to Th2 phenotype
Down-regulate inflammation
Regenerate naive progenitor cells
Trigger apoptosis of dysfunctional cells
Activate parasympathetic (vagal) response, which suppresses spleen activation

Key Insight:
Autoimmunity is inevitable with aging. The immune system is constantly balancing attack and tolerance. Mistakes happen. But abolishing the immune system (like in AIDS) is worse—you get opportunistic infections and cancers. The goal isn't to eliminate autoimmunity but to regulate it.
