The disclaimer is clear: MSC exosomes are not FDA-approved, not standard of care, and there's no guarantee of effectiveness. Every patient signs informed consent. But the safety data? Remarkably clean. Here's what actually happens when you treat thousands of patients.

The procedure breakdown: Nasal and IV are the most common (systemic effects). Fingers, knees, shoulders, and musculoskeletal treatments follow. Miscellaneous applications include intra-articular, trans-tenic, inhaled, and bronchal delivery. Over 2,200 procedures and counting. Adverse reactions total less than 1%: three vasovagal responses (fainting from needle anxiety—treatable with ammonia salts), one knee infection (could happen with any injection), two non-thrombotic vein swelling (one day duration, no explanation), three severe herpes reactivation (oral or genital, manageable with antivirals), one petasis rash (during extreme stress, possibly unrelated), two URIs (immune system shift from surveillance to regeneration), three serious re-injuries during recovery (patient felt great, exercised too hard), one spinal headache (procedural complication, not exosome-related), one persistent injection site pain (first time in five years). Zero allergic reactions. Zero anaphylaxis.

What's NOT an adverse reaction: Vasovagal episodes (happens with blood draws), new injuries from over-exercising during recovery (patient education issue), pain relief leading to premature activity (same), spinal headaches from lumbar puncture (procedural, not exosome-related), drowsiness for 72 hours (normal, patient should sleep), controlled herpes reactivation managed with antivirals (immune shift, not adverse), URI exacerbation in acute illness (timing issue—cancel if patient has fever). Compared to Tylenol, saline, or placebo, exosome adverse reaction rates are lower than the baseline.

Response rates and expectations. About 70% see improvement, but "improvement" is relative. Highly effective: acute injury, inflammation, post-op burns, gout flares, orthopedic surgery within 10 days, accidents (falling from trees/horses). Moderately effective: mild osteoarthritis, autoimmune conditions. Usually effective: facial rejuvenation (as adjunct), erectile function, tendonitis. Intermittently effective: nerve damage (sometimes regenerates, sometimes doesn't—but we have nothing else for this). Less impressive: hair restoration (Dr. Park suspects dosing or protocol issues). The key: manage expectations. People rarely return to being 15 years old. If pain goes away, that doesn't mean full function returns. Attribution bias is real—patients forget timelines, credit other treatments, or expect 100% recovery when 50% is remarkable for chronic conditions.

Dosing guidelines: The brand Dr. Park uses is 800 times more potent than competitors (see Quantifying Exosomes video). Dosing: 1-3 billion for single tooth, hand, or tendon; higher for catastrophic injuries (multi-organ failure, DIC, massive trauma, heart attack, stroke). General rule: a little helps, a lot may not help more—but no one really knows. Safety profile is excellent; knock on wood, no serious complications in five years.
