Hair loss affects up to 80% of Caucasian men. Yet it remains largely untreatable beyond surgery. Dr. Ed Park explores the science of hair, aging, and why exosomes might offer hope.
The Theory:
Hair loss is fundamentally about stem cell depletion. In the hair follicle bulge, hair follicle stem cells (HFSC) and melanocyte stem cells (MSC) reside. When these differentiate rather than asymmetrically divide, they lose their regenerative capacity. This is why chemotherapy causes hair loss and why children with progeria (rapid aging) are bald and gray.
Hair Growth Cycles:
Every hair cycles through anagen (growth, 2–6 years on scalp), catagen (transition), and telogen (rest). The length of each phase determines final hair length. Male pattern baldness involves androgens, DHT, inflammation, and stem cell depletion—but the exact mechanism remains unclear.
Conventional Treatments:
Minoxidil: Mechanism unknown, requires chronic use
Finasteride: Reduces DHT, causes erectile dysfunction
Hair transplants: FUT (strip, scar) or FUE (punch extraction, better cosmetics)
Clinical Cases:
JB (66): After exosome injection, brief hair growth (~8 weeks) but didn't persist. Seborrheic dermatitis improved temporarily.
KW (62): Underwent extensive exosome treatment. Two months later, scalp microneedling (2.5mm) with 2 billion mesenchymal + 2 billion amniotic exosomes. Initially sparse growth, but when he restarted minoxidil, hair significantly improved. Was it exosomes, minoxidil, or synergy? Unclear.
AD (66): After scalp microneedling with exosomes, minimal hair growth.
Why Exosomes Might Work:
Anti-inflammatory effects
De-differentiation of dormant stem cells via microRNAs
Altered hair cycle duration
Apoptosis of dysfunctional cells
Honest Assessment:
Dr. Park has treated only 4–5 patients—far fewer than clinics mandating 3+ treatments. Hair loss is multifactorial, and results are inconsistent. Multiple treatments may be necessary, but the optimal protocol remains unclear. Not a guaranteed cure.
