In this video, I will teach you about Alpha-1 Agonist pharmacology, including their mechanism of action, types, indications, adverse effects, contraindications and special considerations.
Alpha-1 agonists bind to alpha-1 receptors on smooth muscle, causing vasoconstriction, pupil dilation, and contraction of the urinary sphincter. There are three types: phenylephrine, midodrine, and oxymetazoline. They can treat hypotension, nasal congestion, eye redness, orthostatic hypotension, and urinary incontinence. And they can also dilate the pupils for eye exams and procedures. They can cause adverse effects such as bradycardia, hypertension, rebound nasal congestion, urinary hesitancy, insomnia, nervousness and paraesthesia. Some alpha-1 agonists should not be used with urinary retention, acute renal disease, severe organic heart disease and persistent supine hypertension. IV phenylephrine requires close monitoring of the IV site for extravasation, as it can cause necrosis and heart rate for bradycardia. Avoid Midodrine 4 hours before bed to avoid supine hypertension.
