In this video, I will teach you about carbonic anhydrase inhibitor pharmacology, including their mechanism of action, types, indications, adverse effects, contraindications and special considerations.

Carbonic anhydrase inhibitors block an enzyme called carbonic anhydrase, reducing bicarb, sodium, and water reabsorption in the proximal convoluted tubule and aqueous humour production in the eye. Carbonic anhydrase inhibitors include acetazolamide, methazolamide, dichlorphenamide, brinzolamide and dorzolamide. Most end in -zolamide. They are commonly used to treat glaucoma, edema, altitude sickness, kidney stones, and certain seizures. Systemic use of these drugs can lead to hypokalemia, metabolic acidosis, hepatic necrosis, and aplastic anemia. Topical use may also cause local irritation and an unusual or bitter taste. They should not be used with significant liver or kidney impairment, hypokalemia and metabolic acidosis. Routine bloodwork may be required to monitor for adverse effects. Individuals with a sulfa allergy may experience a cross-reactivity reaction as these contain a sulfa moiety.
