In this video, I will teach you about Osmotic Diuretic pharmacology, including their mechanism of action, types, indications, adverse effects, contraindications and special considerations.
Osmotic diuretics directly increase blood and tubular filtrate osmolarity or solute concentration, resulting in aquaresis. Mannitol is the most common and is administered intravenously. They are most commonly used to treat cerebral edema associated with elevated intracranial pressure or ICP and acute glaucoma. Osmotic diuretics can cause hypovolemia, dehydration, hypotension, hypernatremia and hypokalemia in those with normal kidney function and hypervolemia, pulmonary edema, hyponatremia and exacerbated hypokalemia in those with impaired kidney function. They should not be used with anuria, severe dehydration and active intracranial bleeding. If mannitol crystalizes, a new bottle should be obtained. Orders for mannitol often state to hold the dose if serum osmolality is greater than 320mOsm/kg. Report urine output less than 30-50ml/hr to the MRP as the medication may require reevaluation.
