In this video, I will teach you about Beta-1 Agonist pharmacology, including their mechanism of action, types, indications, adverse effects, contraindications and special considerations.
Beta-1 agonists bind to beta-1 receptors primarily located in the heart, increasing heart rate and contractility and, ultimately, cardiac output. Dobutamine is the most common. It is used short-term for cardiac decompensation, such as after cardiac surgery, severe heart failure and cardiogenic shock. The most common adverse effects are hypertension, dysrhythmias such as tachycardia, premature ventricular contractions, and chest pain. Beta-1 Agonists should be used cautiously with tachydysrhythmias such as atrial fibrillation and chest pain. Heart rate and rhythm should be monitored using bedside telemetry, as well as blood pressure, urine output, and serum potassium levels, as hypokalemia can occur. Pulmonary capillary wedge pressure, cardiac index, and central venous pressure may also be required. And finally, before starting this medication, it is important to correct hypovolemia.
